2. Mail in with a check - 2013 CHSA Membership Form (PDF) Print form and mail with check.
3. Pay by check at CHSA Affiliated Horse Show - 2013 CHSA Membership Form (PDF) Print form and bring check to CHSA Affiliated Horse Show.
